Welcome, we’re glad you’re here! Taking an active role in your health care is a big step. The process of moving your health care from pediatric to adult health care systems is called “transition.”

The goal of this quiz is to help you identify areas to focus on to ensure a smooth transition of your LFS care. There are no right or wrong answers and the quiz will take you 5-10 minutes to complete. Your answers are anonymous. This quiz was developed based on health care in the U.S.; therefore, some topics may not apply to people living outside the U.S.
